proračune diljem svijeta. Brojne europske države pri odlučivanju o financiranju lijekova

se interpretaciju koriste jasno definiranim pragovima troškovne učinkovitosti. Bez obzira na

Hrvatskoj prag troškovne učinkovitosti nije određen ni dosad empirijski istraživan te zbog
toga nije moguće interpretirati ekonomske evaluacije, odnosno odrediti kad je lijek u
Hrvatskoj troškovno (ne)učinkovit. Cilj je ovog rada istražiti monetarnu vrijednost jedinice
zdravstvene koristi (različito definirane) kao osnovu za informiranu raspravu o visini praga
troškovne učinkovitosti u Hrvatskoj i tako pridonijeti široj raspravi o povećanju troškovne
učinkovitosti izdvajanja za lijekove iz javnozdravstvenog proračuna.
Pri provedbi istraživanja primijenjena je metoda spremnosti za plaćanje. Spremnost za
plaćanje procijenjena je iz društveno-individualne perspektive i primjenom poreza kao
metode plaćanja. Provedeno je online istraživanje na reprezentativnom uzorku Republike
Hrvatske (N = 1 500) s obzirom na dob (stariji od 18 godina), razinu obrazovanja, spol i
regionalnu rasprostranjenost.
Rezultati pokazuju da su ispitanici za kvalitetom korigiranu dodatnu godinu života (QALY)
spremni platiti 17 000 €, što je jednako 1,15 bruto domaćeg proizvoda po stanovniku.
Ispitanici su relativno više vrednovali dobitak u zdravstvenom stanju u djece (u odnosu na
dobno neutralnu skupinu i starije od 67 godina) te dobitke u zdravstvenim stanjima u kojima
je kvaliteta života relativno niža. Na temelju rezultata istraživanja prag troškovne
učinkovitosti u Hrvatskoj mogao bi biti jedinstven za sva zdravstvena stanja, neovisno o
težini bolesti ili dobi pacijenata (17 000 €) ili bi se pragovi mogli razlikovati s obzirom na
težinu bolesti i dob pacijenata. U potonjem slučaju njegova vrijednost mogla bi biti od 16
777 € za lakša zdravstvena stanja do 20 308 € za najteža zdravstvena stanja u djece.
U radu se također istražuje vrednuju li ispitanici zdravstvena stanja na kraju života ovisno o
tome o kojoj se vrsti bolesti radi (karcinomu ili rijetkoj bolesti). Prosječna spremnost za
plaćanje za dodatnu godinu života na kraju života (EoL-VOLY) iznosi 67 000 €, što je
jednako oko pet puta bruto domaćeg proizvoda. Pokazalo se također da ispitanicima nije
važan tip bolesti na kraju života.
Ovo istraživanje predstavlja prve procjene spremnosti za plaćanje po QALY-u i EoL-VOLYu
u Republici Hrvatskoj, ali i u ovom dijelu Europe općenito. Cilj je pomoću rezultata
dobivenih u ovom istraživanju pridonijeti informiranoj raspravi o visini praga troškovne
učinkovitosti u Hrvatskoj i tako pomoći pri odlučivanju o financiranju lijeka iz
javnozdravstvenog proračuna koje se temelje, između ostalog, na rezultatima ekonomskih
evaluacija. Less Abstract (english) Financing innovative medicines is a challenge for limited public healthcare budgets
worldwide. In many European countries, economic evaluations with clearly defined costeffectiveness
thresholds are used to interpret decisions on the funding of pharmaceuticals
(especially patent-protected medicines). Despite the widespread use of economic evaluations
when deciding on the financing of medicines form public healthcare budgets (and

... More thresholds), the cost-effectiveness
threshold has not been empirically researched in Croatia so far, making it impossible in
Croatia to interpret economics evaluations or determine when a medicine is cost-effective or
not. The aim of this study is to investigate the monetary value of a (differently defined) unit
of health benefit as a basis for an informed discussion on the level of the cost-effectiveness
threshold in Croatia and thus to contribute to a broader debate on increasing the cost-effective
spending on medicines from the public healthcare budget.
The contingent valuation method was used in the research. Willingness to pay was assessed
from a societal-individual-inclusive perspective using taxation as a payment method. An
online survey was conducted among a representative sample from the Croatia (N=1,500), in
terms of age (over 18 years), education level, gender, and regional distribution.
The results show that respondents are willing to pay €17,000 per quality-adjusted life year
(QALY), which is approximately 1.15 times the gross domestic product per capita.
Respondents rated health gains relatively higher for children (compared to an age-neutral
group and people over 67) and health conditions with a relatively lower quality of life. Based
on the research findings, the cost-effectiveness threshold in Croatia could be uniform for all
health conditions, regardless of the severity of condition of the age of the patient (€17,000),
or it could vary depending on the severity of the condition and the age of the patients. In this
case, the value could range from €16,777 for milder health conditions to €20,308 for the most
severe health conditions in children.
The research also examines whether respondents value end-of-life health conditions
differently depending on the type of disease (cancer or rare disease). The average willingness
to pay for an additional year of life at the end of life (EoL-VOLY) is €67,000, which is about
five times the gross domestic product. It was found that respondents make no distinction
between the types of illness at the end of life.
This research represents the first estimates of willingness to pay per QALY and EoL-VOLY
in Croatia and Southeast Europe in general. The results of this study can contribute to an
informed discussion on the level of the cost-effectiveness threshold in Croatia and thus help
in decision-making on the financing of medicines from the public healthcare budget based,
among other things, on the results of the economic evaluations. Less Keywords
